Association among Insulin, IGF-1 and IGFBPS with Risk of Colorectal

[Abstract]

Objective:To study the association among the changes of serum Insulin,Leptin, Insulin-like growth factor(IGF-1),Insulin-like growth factor binding proteins(IGFBPs)levels and the values of IGF-1/IGFBP-3,the changes of Body mass index(BMI),Waist and hip circumference ratio(WHR)and the genesis of colorectal cancer.Seeking for the invasion cause of colorectal cancer provides scientific evidence for its precaution,treatment and control.Methods:Sera from 244 colorectal cancer patients before the operation,371 patients after the operation and 150 healthy subjects were assayed for Insulin,Leptin,IGF-1,IGFBP-1,IGFBP-3 and IGF-1/IGFBP-3 by the enzyme-linked immunosorbent assay and radioimmunoassay. SPSS13.0 statistics software was applied to analyze the data.Results:The serum values of Leptin,Insulin,IGF-1 and the ratio of IGF-1 and IGFBP-3 in colorectal cancer patients before and after surgical treatment were significantly higher than in controls.There were significant differences in the five groups(P=0.035, P=0.021,P=0.042,P?0.001 respectively).The ratio of IGF-1 and IGFBP-3 in patients before surgery was significant higher than that in patients after surgical resection,and the difference has significance.The serum levels of IGFBP-3 in patients before and after operation were significantly lower than in controls,and the differences have statistic significance(P=0.015, P=0.001,respectively).The serum level of IGFBP-3 in patients before surgery markedly lower than that in patients after surgery,and the difference has significance(P=0.023).The serum values of IGFBP-1 in patients before and after surgical treatment were close to healthy controls, the differences have no significance(P?0.05,respectively).The serum levels of Insulin,IGF-1 in colorectal cancer patients after operation within one year,within 3 years and over 3 years were significantly higher than in healthy volunteers,and the differences have significance(P=0.032,P?0.001,P=0.029 and P?0.001,respectively).The serum IGF-1 level in patients before operation was not significantly different from those in patients within a year,within 3 years and over 3 years postoperatively(P?0.05 respectively). The serum IGFBP-1 levels in postoperative patients within one year,within 3 years and over 3 years were close to that in healthy subjects(P?0.05 respectively).The serum values of IGFBP-3 in patients after surgery within a year and 3 years were markedly lower than healthy controls,and the differences have significance(P=0.023,P-0.005 respectively).The values of IGF-1/IGFBP-3 in postoperative patients within one year,within 3 years and over 3 years were higher than in healthy controls,and the differences have significance(P?0.001,P=0.001 and P=0.002 respectively).But the value of IGF-1/IGFBP-3 in postoperative patients within one year, within 3 years and over 3 years were lower than in preoperative patients,and the differences have significance(P=0.020,P=0.034 and P=0.017,respectively).The serum Leptin,Insulin levels of colon carcinoma patients were lower than those of rectal cancer patients,but the differences have no significance(P=0.093,P=0.134 respectively).The serum IGF-1 and IGFBP-1 levels of colon carcinoma patients were significantly lower than those of rectal cancer patients,and the differences have significance(P=0.034,P=0.031 respectively). The serum IGFBP-3 level in colon carcinoma patients has no difference from that in rectal cancer patients(P?0.05).The serum value of IGF-1/IGFBP-3 in patients with colon carcinoma was significantly lower than in rectal cancer patients,and the difference has significance (p=0.035).There was no obvious difference in serum levels of Leptin,Insulin,IGF-1,IGFBP1, IGFBP3 and the value of IGF-1/IGFBP3 between colorectal cancer patients after surgical treatment within one year,with 3 years and over 3 years who did not have distant metastasis and patients with liver and abdominal cavity metastasis(P?0.05).The BMI in colon carcinoma patients(23.57?4.26kg/m~2)and the BMI in rectal cancer patients(23.04?3.32kg/m~2)were no difference to the healthy controls(23.36?3.06kg/m~2) (P?0.05).The WHR in colon carcinoma patients(0.99?0.07m)and the WHR in rectal cancer patients(0.89?0.06m)were higher than in healthy subjects(0.82?0.05m),and the differences have significances(P=0.003,P=0.035 respectively).The WHR in colon carcinoma patients is different to the rectal cancer patients(P=0.046).The BMI in colorectal cancer patients was positively correlated with the WHR in those patients(r=0.523,p?0.001).The WHR and BMI in colon carcinoma patients were positively correlated with the serum Leptin,Insulin levels and the value of IGF/IGFBP3,and the p values were p=0.001,p?0.O01, p?0.002,p=0.045,0?0.05 respectively.The WHR and BMI were reversely correlated with IGFBP3(9=0.005,p?0.018 respectively).The WHR and BMI were not correlated with IGF-1 and IGFBP1.The BMI in rectal cancer was positively correlated with the serum IGFBP-1 level(9=0.016). Tile WHR and BMI were not correlated with the serum IGF-1,Leptin,Insulin levels and the value of IGF-1/IGFBP3(9?0.05).The BMI in colorectal cancer patients was positively correlated with the value of IGF-1/IGFBP3(9=0.041).The BMI and WHR were not correlated with the IGF-1,IGFBP-1,IGFBP3,Leptin,and Insulin levels(9?0.05).The serum CEA level in colorectal cancer patients was negatively correlated with the Insulin level(r=-0.233,p=0.05).The CA19-9 and CA242 levels were negatively correlated with the IGFBP1 level,the values were r=-0.276,p=0.021 and r=-0.256,p=0.041 respectively.The CEA,CA242 and CA724 were negatively correlated with the serum IGFBP3 level,the values were r=0.305 p=0.01,r=0.253 p=0.034 and r=0.332 p=0.005 respectively.The CA724 was negatively correlated with the value of IGF-1/IGFBP3(r=-0.233 p=0.045).Conclusions:The serum Leptin,Insulin,IGF-1 levels and the value of IGF-1/IGFBP-3 were significantly increased in colorectal cancer patients and serum IGFBP-3 level was markedly decreased in patients,which may related to the genesis of colorectal cancer.Serum levels of Leptin,Insulin,IGF-1,IGFBP-1,IGFBP-3 and the value of IGF-1/IGFBP-3 were not correlated with the progress and improvement of colorectal cancer.The BMI was not correlated with colorectal cancer patients.WHR in colon carcinoma patients was significantly higher than in healthy controls and in rectal cancer patients.Central adipositas may be a risk factor for the genesis of colon cancer.Serum levels of Leptin,Insulin,IGF-1,IGFBP-1,IGFBP3 and the value of IGF-1/IGFBP-3 were correlated with tumor markers such as CEA,CA19-9,CA242,CA724,but the correlation is irregularly.
